Harbor of Health provides structured outpatient substance use disorder care for adults in Philadelphia, combining counseling, individualized treatment planning, care coordination, and medication-assisted treatment support.

One coordinated plan

Treatment addresses more than substance use alone

Recovery can be affected by cravings, physical health, trauma, relationships, housing, transportation, employment, and access to medical care. Harbor begins with a careful assessment and builds an individualized plan around the needs that matter most to each person.

Services are delivered in an outpatient setting. Patients return home after appointments and can continue work, school, and family responsibilities while participating in care.

Harbor primarily serves adults age 18 and older with opioid, alcohol, stimulant, cannabis, or polysubstance use disorders when outpatient care is appropriate.

Assessment & treatment planning

Screening, biopsychosocial assessment, level-of-care review, individualized goals, and regular plan updates.

Individual & group counseling

Structured counseling focused on recovery skills, triggers, coping, relapse prevention, and sustained progress.

MAT support

Medical evaluation, medication coordination, monitoring, and onsite Vivitrol administration when clinically appropriate.

Care coordination

Referrals and coordination with medical, behavioral health, recovery, and community support services.

Discharge & aftercare

Transition planning, recovery-support connections, follow-up, and a written plan for continued progress.

Clinical monitoring

Progress review and toxicology screening when clinically indicated and incorporated into the treatment plan.

Harbor of Health does not provide overnight care, emergency stabilization, or inpatient detoxification. A clinical assessment helps determine whether outpatient treatment is appropriate. When a higher level of care is needed, staff can help coordinate a referral.
