Patients and authorized parties can contact Harbor for current medical-record request instructions. Substance-use treatment records may require a specific written authorization.

Basic request process

1. Contact Harbor

Ask for the current records-request or release-of-information form and secure submission instructions.

2. Complete the authorization

Identify the patient, recipient, records requested, purpose, expiration, and other required elements.

3. Verify identity or authority

Provide any documentation reasonably required to confirm identity or legal authority.

4. Submit securely

Use the current secure method provided by Harbor and keep a copy of the request.

Important confidentiality note

Substance-use disorder treatment records can receive additional federal confidentiality protection. A general HIPAA authorization may not always be sufficient. Harbor will review the request and explain any missing information or required authorization.

Processing time depends on the request, applicable law, identity verification, authorization, record volume, and whether another party is involved.

Do not send records or signed authorizations to the general website form or ordinary email. Call first for secure instructions.

Urgent clinical continuity

If a treating provider needs information for time-sensitive continuity of care, call Harbor and clearly explain the urgency. Emergencies should be directed to 911 or the appropriate emergency department.
