Start with a phone call

Call Harbor to confirm current availability, review the requested service, and receive secure instructions before transmitting protected patient information. Whenever possible, involve the patient directly and confirm a reliable way to reach them.

1. Confirm fit

Share the general reason for referral without sending unnecessary protected information through ordinary email or a website form.

2. Obtain authorization

Secure a valid patient authorization when required before releasing substance-use treatment or other protected records.

3. Send records securely

Use the current secure method provided by Harbor—not a general website inquiry or unsecured email.

4. Coordinate next steps

Harbor contacts the patient, completes screening and assessment, and determines the appropriate level of care.

  • Reason for referral and current level-of-care concern
  • Recent substance use and withdrawal-risk information when securely transmitted
  • Relevant medical, psychiatric, medication, and treatment history

When a different level of care is needed

Harbor provides outpatient treatment and does not provide emergency stabilization, inpatient detoxification, residential treatment, or overnight care. If assessment indicates a higher or different level of care, Harbor can help coordinate a referral.
